Sunset Harbor Homes

Who We Are • What We Do

Sunset Harbor Homes offers round-the-clock short-term (less than 90 days) and long-term transitional care programs, delivering comprehensive inpatient care to individuals of all ages. Our highly skilled and compassionate staff provides an array of medical and rehabilitative services tailored to meet the needs of each patient. Additionally, caregivers coordinate with Home Health providers and other community health resources to ensure continuity of care following discharge. Care programs and services are led by an interdisciplinary team of health care professionals including, but not limited to, primary care physicians, rehabilitation specialists, pharmacists, dietitians, case managers and clinical care coordinators.

Sunset Harbor HomeCare

HomeCare is the newest addition to our services. It provides non-medical in-home care to seniors, disabled adults, those recovering from surgery, illness, or injury, or anyone who needs help with the activities of daily living.
